Talking Death Heads
When you die, you speak from your head! Known issues: can be a decent amount of clipping, no text to speech (I heard of the mod DeadTTS which might work with this?), and a slight delay after the arena fight before everyone can talk again.

When you die, you speak from your head. You watch your head. Your friends find the head in a dark corner, crying for help, begging to be taken back into the light.
Known issues: no text-to-speech (DeadTTS might work with this? Haven't tried yet...), bit of clipping, a very small delay before the winner of the arena can talk with folks in the lobby again.
