Hold ' to scratch your bum (撓屁股). Look at another player up close to scratch theirs: they jump to the game's snatch sound and pull a random face. Random eyes on every press. Players with the mod see it all.
First person: when you scratch your own bum, the hand you see now moves to your bum as well (before, only your body and its shadow did).

Default arm pose and aim now match the values the mod was tested with: elbow elevation 37 (was 30), elbow radius 2.0 (was 1.4), aim angle 20 (was 25).
If you already started the game with 1.0.0, your config keeps the old values: change them in BepInEx/config/bigwalk.scratch.cfg, or delete that file to get the new defaults.
